Please consider the following to determine the type of stain

It looks like nails or similar iron items were laying on the plaster. Have you tried to rub a chlorine puck on it?

It could be. There is to be 3 inches of gunite above rebar plus another inch or more of plaster. It is not unheard of to be rebar but I was looking at the obvious things first such as items laying on the plaster.
