For starters, crumple that up and throw it in the trash.
Pool store testing is usually garbage (and their "correct" range is completely wrong).
Next, get a complete test kit, we recommend the K-2006C, or the TF-100 (the TF-100 has the absolute best value for the amount of testing reagents you get - you can find it here:
TFTestkits.net ). Once that test kit comes in, get us a complete set of test results:

Based on what I'm seeing from the store test, your CYA is at least 100, if not higher. This means you're going to need to drain the pool at least 50% to get it under control. With the heat and sun we're now having in AZ, this may have to be done carefully over time. Once your kit is in, we'll be able to get a good CYA reading and give you better advice.
While waiting for your kit, add a half gallon of plain, unscented concentrated bleach a day into the pool (make sure it is not splashless or EZ-Pour). This might help to keep you from getting into worse trouble.
Remove any and all solid chlorine products - pucks, powders, etc from the pool immediately to avoid raising that CYA any higher. You're going to want your pool to be on a bleach only diet until we get some reliable test results.
