What to do about my pool return?

Hi All,
I am trying to figure out what to do bout my water return? As you can see from the pictures it's falling apart. Is there a way to patch this, or does the whole need to be jackhammered out and remade? Is that even possible?

That looks like a skimmer, not a pool return.

The concrete around the skimmer is cracking and needs to be replaced.

The condition of the skimmer itself is not clear. Pull out the basket and post pics of that the skimmer body looks like.

What does the mouth of the skimmer looking in from the pool look like?
