Pool Math Windows application lost my subscription

I generally run PoolMath on my iPad, where it continues to run fine.

Today I wanted to enter a log from my Windows 10 PC and it can't find my subscription. Attempting to Restore Subscription also fails. You can see multiple logs from my iPad in this Windows screen shot.

I logged out but cannot remember my credentials. When I attempt to recover them, it tries to go to this page, with bad results:

I eventually remembered my credentials, and the PC app now works again. But that login.php page seems broken.
