Can someone please help me with figuring out what model Praher valve i have?

Hey folks, after determining that i was losing a significant amount of water out of my waste/backflush line, I determined that the valve needs a good rebuild. I thought it was a Heyward sand filter and ordered what I thought was the correct rebuild kit. Well today I decided it would be a good day to tackle the job. After getting the top of the valve off, I realized I got the wrong stuff.

Turns out that I don't have a Heyward filter, but it is a Ranger Series Sand Filter from cooperpool.com.

Please help me figure out which model it is so I can search for the correct parts accurately... hopefully the attached pics is enough to help.

The 1.5" port Praher Top Mount Multiport Valve 6-Way - TM-12-L will also fit the CLAMP DOWN 8.25” CLAMP FLANGE if you want to replace the whole valve.

Thank you, thats very helpful. I didn't see any additional information other that what remains on the sticker. I'll try to look closer tomorrow. I will see what I can see on the web what the the internet as I sit here watching football. I do know for sure that both the inlet/outlet are 2 inch and the waste/backflush outlet is 1.5 inch
 
The multiport is a Praher TM-22-L.

Pentair lists it as a 14971-TM-22-L.

All ports are 2".
