I put it on making sure to push the pegs in, and it looked set.

thanks wireLooking at the peg @JamesW posted you can see the peg was not pushed in past the catch lip of the peg. You can warm the flap with a hair dryer and reshape it. Let it cool and push the nub/peg completely in so it can't just back out.
YupWhen my new one arrived, it wasn’t completely flat. It has a little curve to it.
