Pebble Sheen Plaster Formation

Any idea what might cause this? The this volcano looking structure has formed on the pebble sheen plaster in the shallow end of our pool. The plaster is 2.5 yrs old and Ca levels have always hovered around 300ppm. Its hard like calcium but has a cavity in the middle (like a crater). its maybe half an inch in diameter and protrudes about half an inch from the plaster.

Thanks so much for the suggestion. That sounds exactly like what I have. I may carefully take pumice stone to see if I can take it down and see if it comes back.
