Help determining Pentair multiport model

I have an older pool and want to replace the Pentair multiport 6 way top assembly. I have a leak under the handle and some leakage out of the drain pipe when on filter. The problem I have is that there are no model numbers on this and I need help figuring out what to order. Attached is a picture of what I have. The pipes connected to this are 1 1/2 inches if that helps. This site is awesome and very helpful. I searched the forums, but couldn't find information on this.

What type and model filter do you have?

It's a fairly large sand filter. Not sure of model. It's round shaped and brown and looks like made of fiberglass. It came with the house when we purchased it several years ago.
