Just a thought... I don't know how many bubbles it would produce, but maybe you could fish aquarium tubing down through the main drain pipe, pull it out through the main drain cover and attach an aerator stone. Then use a fitting in the main drain pipe similar to what is used for a Stenner pump...

Yep a tub/shower diverter valve would be the easiest way to go, especially if you want to keep the plumbing in the wall. Just make sure you have proper drainage away from the house.

I've seen lots of neat open-air showers. When you plumb it, include a low spigot that can be used for a foot rinse. I don't know if you ever have freezing temperatures where you are, but that will also allow you to drain down the pipes if necessary.
