What pieces make up this slide valve?

I’m trying to figure out what pieces make up this slide valve? It sticks pretty good and the threaded portion is cracked. Might as well replace the entire thing at this point. Thanks for your help.

Slide valves stink. You should replace it with a multiport valve.
