Sta Rite Max E therma 400 shutting off

Hard to tell with basically zero information about what you have.

Maybe the safeties are bypassed or defective.

All we can do is speculate about 100 different things that might be wrong with the heater.

Seems like a disaster waiting to happen.

Update. I took the manifold off and the back plate is melted. The manifold seems to be fine though. What could cause this?
Unfortunately, that's why those of us in the industry generally won't buy or install used equipment, especially heaters. Mixing 400,000btu of gas with electricity and flame, along with one's property and family, usually only gets one chance to do it right.
It could have been that way when you purchased it despite being told it was working. There is always a reason a piece of equipment is removed from a system and it usually isn't a good one.
Have never seen that kind of damage on a StaRite/Pentair heater. It could be a cracked burner letting uncontrolled flame get into areas it shouldn't. It's possible that a safety in the heater failed and the heater stayed lit with no water going through it. It would take a complete teardown, and a complete check of all the safety systems in the heater after replacing everything, to check it for that kind of damage. Sorry.

It seems to me that the blower would need to be off or running too slow to allow the temperature to go that high.

Maybe flame with no blower or a weak blower or maybe no water flow.

Low voltage to the blower can cause the blower to run too slow.

Most likely one or more of the safeties like the PS, SFS, HLS, AGS, AFS etc. are defective or bypassed or the system would have shut off and given an error.

The following are the LEDs:

  1. .....PS (Pressure Switch)
  2. .....HLS (High Limit Switch)
  3. .....SFS (Stack Flue Sensor)
  4. .....AFS (Air Flow Switch)
  5. .....AGS (Automatic Gas Shutoff).
  6. .....Service System
  7. .....Thermistor
  8. .....Heating
  9. .....Pool On
  10. ...Spa On
  11. ...Service Heater.
 
We do not know the age or condition of the heater and we do not know if it was installed correctly.

Maybe you have 2 psi of gas pressure going into the gas valve.

Maybe you have a propane model.

Maybe the voltage is incorrect.

It might be one issue and it might be 10 issues.

Maybe you have the plumbing backwards.

There are far too many unknowns for us to be able to do anything more than speculate and throw out an endless number of possible problems.

Most likely, this is a total loss and just take it as a lesson learned.

If you want a heater, buy a new one and get it professionally installed.
