I would also try completely draining the spa and letting the plaster dry, plugging all the returns in the pool then turn on the pump to return to the spa only and look for the wet spot in the plaster.
Who installed the check valve, there should be a arrow showing direction of flow. Make sure the flow is from pool return to spa return. If you turn valve 180 degrees to spa return with pool suction on what happens?
I also thought about the check valve being in backwards, but assumed that would only make a difference in the Pool mod.. in the Spa mod all the water should by pass the check valve anyway.. My initial thought was that it is more likely that something is screwed up at the pad rather than something being plastered over, but I can't come up with a logical answer as to what...
