Help with diving board

Hey everyone, new member here, I enjoy reading the forums and learning from all of you about how to maintain this new (to me) pool.

Somewhere along the line, the diving board was broken before we bought the house, but the base was left in the pump shed. Could anyone help to identify what brand this could be? I've tried many combinations of search terms, but haven't had any luck on my own so far. There are 5 bolts in the concrete, 3 at the back and 2 in the front.

Are there any names on the fiberglass portion? Most were done by SR smith...

There was a sticker on the base with SRC88 on it. When I Google that it brings up 3 and 4 bolts, but not the 5 that are here.

Does the pattern fit for the 4 bolt ones ? The PB could have added the middle hole/bolt in the back of he felt it needed more support.
