How did pressure get locked in the heater?

Did the heater run while all valves were closed?

This is why all heaters need a real "Flow" switch and not just a "Pressure" switch.

The output should not be able to be blocked by a valve.

If you use a check valve, then pressure cannot be locked in.

What was the pressure rating on the relief valve?

"The relief pressure of the valve MUST NOT EXCEED 50 PSI".

"Important! The relief valve should be tested at least once a year by lifting the valve lever".
