Single Broken Tile - Looking for replacement

had few tiles pop out over the winter and one was cracked down the middle. Looking on line I just cant find a match - some look close but not exactly right.

It is a 3x3 inch. Anyone recognize the style and a have a name or a dealer that they know sells it?
