Help with Return Jet Fittings

I have confirmed that I have one return jet leaking right near the termination to our pebbletec pool. For some reason, the plumbing on this return is different from all the others, leading me to believe it's either original (1980's pool) or has been replaced. Either way, I'm trying to figure out if I can repair it without paying +$1k for Red Rhino to do a full cut back and new return install. But more importantly I don't want to deal with them mismatching our finish and having to drain our pool. When I put my hand over the fitting, water comes out from behind the face plate (where there was previously epoxy). Red Rhino confirmed it is on the pool side of that odd fitting. They tried to do the epoxy job shown but that just made it worse and I have since started to remove it (shown removed in IMG_7368).

1) Does anyone know what the furthest fitting is (away from the camera) with the four half-round shapes inside of it in my photo (DSCF4198)?

2) Does anyone know if I can remove the face plate (assuming to use some sort of wrench in one of the four notches) - DSCF4197 and DSCF4199? i.e. does the faceplate have external threads?

Any help/insight would be greatly appreciated.
