The heater puts out a fixed amount of BTU's. Water flow rate makes no difference in the heat (BTUs) that get into the water. Flow rate will affect the outlet water temperature. Read Heater Maintenance - Further Reading

If the heater is running and hot air is coming out the exhaust then BTUs (heat) is going into the water. No other place it can go. What makes you think otherwise?
