Usually, yes a permit is required unless it's less than 24" tall and no structural frame such as a small blow up pool. In any case it's best to check with the local authority.
100% Depends on the municipality. Some places would let you build a castle without batting an eye, others want a permit if you are putting a new mailbox in.
We don't require permits for anything where I'm located. You could build a space port and not have to have a permit. However the little town to the south of us requires a permit for almost everything, except an above ground pool.
So, as pinguy said, it 100% depends on the local AHJ.
