I'm fairly certain it is fine sand and other particles caught in the filter. I vacuumed the entire pool, turned off the filter and left it overnight. With the pump on it usually results in the problem. After leaving it off overnight, the pool was still crystal clear. Must have a crack in a lateral.

Hard to tell. Mostly along the lines at the bottom of the pool, some in the shallow end, and if theres a little crease in the liner then along that crease

After 2.5 weeks of being at 1.0-1.25x shock level and no improvement I got desperate and went 2x to see if there was an improvement.

Has definitely cleared up. Not crystal clear but only a slight tinge of green. Definitely improved. FC is back down to around 22.
