Polaris wall fitting issue

What causes the problem in the picture attached? The wall plug starts to come out on one side, then eventually blows off into the pool.

Perhaps too much pressure and the quick disconnect end wasn't clicked into place entirely? In case you need parts:

Polaris Universal Wall Fitting Collar (UWF) - 6-500-00
Polaris 6-502-00 65/165 Wall Fitting
Polaris Pressure Relief Valve 6-503-00

Do you have another thread started about cleaning up that pool?? Need any help?

Thanks for the help! I did. I've just been traveling and have two clogged skimmer pipes from kids not putting baskets back on right. So gotta get that fixed so I can get it vacuumed up!
