Help to ID Multiport Valve

The following are the photos of my multiport valve. The handle no long stays upright, it often leaks water out of the drain when in filter mode, and it gushes water out of the drain when in recirculate mode. My thinking is that with a proper ID, I can buy new gaskets and the parts to kind of refurbish it properly. Any help would be greatly appreciated. Here goes....

That looks like the multi-port that comes with a Pentair Tagelus/Pac-Fab TA-60 filter.

The filter is a Tagelus TA-60 and on the side of the valve it has the manufacture date of 04/98. But, I was thinking I had to know the valve part number to order the parts or are you indicating this is like a "combo" deal where all TA-60 filters came with this multiport valve from the factory? I just need to find some parts. lol

There have been two, or maybe three, changes to the valve design over the years, but yes all of those filters came with the same valve in any given year. The differences between years are fairly easy to spot and then you lookup the parts somewhere like here.
