Repairing Slide Backwash Valve

My slide-type backwash valve is leaking water through the backwash hose- with enough pressure to burst the hose!

I can't tell what brand it is- it looks similar to Pentair and Hayward valves, but it has a metal valve stem, and what appear to be O-rings are actually rubber discs with a thick edge (I have attached a photo).

The valve is connected to an Atlas PA-48 DTE filter, and the filter unions appear to be on 8" centers. I've disassembled the valve and greased the rubber parts, but the water continues to flow from the backwash fitting.

Does anyone know what brand of valve this is, where to get parts, or have any suggestions for repairing it other than cutting it out and replacing it entirely? If the only option is replacement, what would be the best choice of part?
