4" floor returns ?

OK

the PB left no screws in the inner "diverter" but I still can't get enough leverage on the top of the disk to get it to spin open

Maybe a special tool ??

The two centered holes are for a tool to grip the disk. Large needle nose pliers will usually work.

Some places sell a tool that has adjustable width pins for such purposes.
 
If you can't get it with needle nose pliers, try an Adjustable Pin Spanner Wrench. Make sure that the pins will fit in the holes (pin diameter and spacing width).

Make sure that the set screw is not in the third outer hole.

You could even make a tool. For example, you could drill two holes in a 2x4 and stick metal pins in the holes and then use that to unscrew the disk.
