Need Help Creating A Swimming Pool Data Sheet

I am in the process of creating a one-page sheet that a customer can print out to write down all the things they need to know about their pool and spa. I want to make one for an above ground pool and an inground pool. All residential. This sheet can help homeowners to keep track of the equipment they have and size of their pool in case they need to reference it for any reason. So far here is what I want to add to the sheet.

If you have any suggestions, they would be much appreciated.

- Homeowner's name, address and phone number
- size of pool: gallons, width & length
- type of filter and brand
- type of pump and brand
- type of chemical feeder and brand
- type of heater and brand
- type of automatic pool cleaner and brand
- how many inlets
- how many skimmers
- diving board: yes or no
- deck: yes or no
- slide: yes or no
- how many in pool ladders
- how many in pool steps

That's about all i can really think of. The point is to make this simple and useful for the homeowner. I'm looking forward to your suggestions. :-D
 
Model # and Serial #'s of equipment would be better that just brand name.
Installation/completion date of pool, or purchase date of equipment items.
On newly constructed pool - name of Installer/builder
 
Instead of making separate sheets for IG vs AG why not just specify pool type (AG Perm, AG Seasonal, IG Gunite/Concrete/Plaster, IG Fiberglass/Composite, IG Vinyl lined)
