I am using a liquid chlorine product called "Ultra Shock" pool shock. The label states available chlorine 15%, sodium hypochlorite 12.5%, the rest is inert ingredients. Which % do I use in the pool calculator? Thanks in advance.

Go with the 12.5%. That's the amount of sodium hypochlorite by weight and that is what you are interested in.
The label must be wrong in some way because the 12.5% with 15% doesn't work. The relationship of different percentages is as follows:
% Available Chlorine by Weight < Weight % Sodium Hypochlorite < % Available Chlorine by Volume (aka Trade %)
For typical 6% bleach and 12.5% chlorinating liquid, this is as follows because bleach is typically listed by the weight % of sodium hypochlorite while chlorinating liquid is normally spec'd by Trade % which is % Available Chlorine by Volume:
5.71% Available Chlorine by Weight < 6% Weight Sodium Hypochlorite < 6.17% Available Chlorine by Volume (Trade %)
10.78% Available Chlorine by Weight < 11.31% Weight Sodium Hypochlorite < 12.5% Available Chlorine by Volume (Trade %)
It's hard to know what they really mean by their label, but as was suggested you should just use 12.5% in The Pool Calculator and see how things go.
