One of my check valves has a crack/leak in the cover. I am having a hard time finding the correct one to see if a replacement cover is available. Does anyone have one like this and do you know what the brand/model is? Thanks.

Can someone tell this novice what a "check valve" actually is / actually does? In case I ever need to know? Thanks.
It only allows the water to flow in one direction. More to the point, it stops water from flowing backward from it's normal flow direction.
It's common pool/spa use is to keep the spa from draining back into the pool when the pump is off. There are other uses, but that's probably the most common.
