Try a 3:1 solution water and muriatic acid (3 waters).

It should foam up on contact, wear gloves, and scrub with a deck brush. Rinse quickly.

I assume you have already tried an abrasive like Ajax and water, right?

well, i will second the part where you say "rinse quickly". muriatic acid is going to eat up the grout, cant leave it on long. I use a 50/50 of diluted bleach and spray it on and let it sit until it dries, and then rinse and scrub with a nylon brush and works great.
 
The "yellowish/orange staining" that OP described didn't sound like algae, which chlorine will remove very, very well.

It sound like calcium buildup which will require some form of acid to remove.
