I know this old fool who has a spa & don't change the water but once a year. He says the water is clear and he balances it. But ain't that a dirty dirty thing? He says he puts his greasy body in it at least 3 times a week. Ugh.

It can be quite unsanitary, actually. I recommend spa owners change their water about every 3 months.

During regular spa use, non-organic contaminants (like perspiration, body oils, personal care products, sunscreen, and worse) will build up in the water. Letting these byproducts, referred to as Total Dissolved Solids (TDS), accumulate in the water will cause sanitation issues. This will then lead to you spending more on chemicals to compensate, or it becomes a little like a giant petri dish.

Changing your water every 3-5 months is generally a pretty good idea.
